3/12/2026 - Next Chapter - Healthcare: Navigating Needs & Medical Transitions

Southbury Senior Services' new educational panel series titled “Next Chapter: Planning Your Path Forward,” designed to help older adults and their families navigate the complex decisions that come with aging and future care planning.

This free, multi-session series is open to area seniors and their adult family members and will feature expert panelists including Elder Law attorneys, Financial Advisors, Home Care agency representatives, Healthcare professionals, Social Workers, Senior Center advocates, and representatives from the Area Agency on Aging.

Each session will provide practical guidance, resources, and compassionate insight into topics such as aging in place, transitioning to care facilities, legal and financial planning, and caregiver support.

Second Session titled “Healthcare: Navigating Needs & Medical Transitions,” is designed to help older adults and their adult families understand how to prepare for changing health needs and confidently move through medical transitions.

As individuals age, healthcare decisions and care coordination often become more complex. This session will provide practical, compassionate guidance on recognizing when additional support may be needed, how to plan for upcoming medical changes, and where to find trusted help during hospitalizations, rehab stays or shifts in medical condition.

Featured Speakers:

  • Tamath Rossi, CDP – Senior Advocate and owner of Supporting Our Seniors LLC
  • Heidi Bettcher, RN BSN a ICP – Healthcare Professional specializing in care transitions and chronic care planning

Together, they will offer expert insight on:

  • Early signs that more support may be needed
  • Navigating hospital > rehab > home transitions
  • Coordinating care and communicating with providers
  • Advocating for yourself or a loved one
  • Preparing for medical changes before a crisis occurs

Each session will include time for Q & A . At the end of the series, comprehensive resource packets will be made available for pick up.
